Random Deaths

I was in a map with the boss Black Crow (Base boss Crowbell), and whenever I would phase him and he would walk into the second arena, I would just randomly die for absolutely no reason. I lost that map, so I went to a different location, and I randomly died again, even when I wasn't hit. I am running Chaos Inoculation so I thought it was a bug with it, but a friend experienced the same issue.

I realize the issue. At base the Berserk spirit gem will leave you on one life, but when paired with Chaos Inoculation, it bypasses it and that causes sudden death from "out of nowhere".
