No Quest Item Act 9 The Refinery

After a game crash, I lost the Quest Item, and since then, I could not get it anymore, and now I'm stuck there, and I already restarted the game, waited a day, and tried multiple new instances, but sadly, I still don't get the Item

It might have already been used. Did you talk to the Sin in The Quarry? What does the quest tell you?
