Weird intermittent FPS issues

So for the last few days now, sometimes my game will show that I have 160+ FPS like normal, but the game looks like it is running at 20-30 FPS. It's super inconsistent and very strange with what does/doesn't fix it, and for how long it stays fixed. If I tab out of the game to my other monitor, then it is at the expected FPS again. However when I tab back in, after about 5 seconds, it will suddenly feel like 20-30 FPS again. One of the main fixes I've found is to swap which monitor the game is on and back, which usually fixes it. However sometimes when I alt tab it will retrigger the FPS "drop", and sometimes I can alt tab just fine till I close the game. Also sometimes restarting PC works, sometimes it doesn't.
I have a Radeon RX 7900 XT, Ryzen 7 7800X3D, and 64 GB of DDR5 4800 MHz RAM. This issue also only occurs with PoE, no other game has this happen even very intensive games such as full Ultra Cyberpunk 2077 and BF6 on full ultra as well. No input lag. Everything plays like it's at 30 FPS like animations are all choppy, input definitely feels slower but that's due to the frame rate. Though like you can see the game still shows normal FPS while it is happening. It also doesn't show in videos or streaming my screen.
With Vsync on, while it is happening the metrics shows the 20 FPS I appear to be getting, and frame time is very high compared to normal. Vsync on, during issue: ![]() Vsync on, normally: ![]() But also after alt tabbing with vsync on, if the issue DOES reoccur, then after a second or two of the issue, it acts like vsync gets turned off and goes back to my original metrics like this. ![]() If it DOESN'T reoccur, then it will stay at the 60 FPS as seen in the second screenshot.
