Omen of Resurgence not activating

TLDR - I died without this omen activating.

It was active. No other omens active. It had not activated during this "combat instance". T15 map, Riverhold, 0.4, league character.

Edit, it happened 3 times in last 10 maps. I did see it proc once in those 10. These 3 times were not one-shot hits from enemies...gradually losing health. Two were from lots of hits, another from bleeding.

Doesn't it trigger from low heath, rather than actual death from being one shot? If you're looking to prevent that, try the more expensive amelioration omen.
